This pre-screening questionnaire must be completed by all vendors expressing an interest in being registered and pre-qualified with The NGC Group in the service category:

Accident and Emergency consultations and treatment (24-hour coverage). Injury consultations and treatment for all levels of emergencies with associated testing (x-rays/ECGs/ultrasounds/CT scans/POP applications and similar type emergency treatments).
Employee Medicals inclusive of diagnostics (blood investigations, radiographic studies, ultrasounds, ECGs, Echocardiograms, mammograms, Pap smears, Stress ECG, urine testing, urine drug testing, breath alcohol testing, Chester step, audiometry, spirometry, respiratory fitting).
Occupational Health testing in accordance with OEUK, CAOHC, NIOSH, NDASA Standards.
